IHA’s Indiana Patient Safety Center (IPSC) presented its 2018 Indiana Patient Safety Awards at the Indiana Patient Safety Summit in June. The annual awards recognize individuals and teams for their commitment to and enthusiasm for improving patient safety in hospitals and health care systems across the state. Learn more about each of this year’s award recipients below.
THE PATIENT SAFETY INNOVATION AWARD
Franciscan Health Hammond received the 2018 Patient Safety Innovation Award for its exemplary work with its Comprehensive Transitions of Care Pharmacy Program. Through this innovative approach, the hospital bridged gaps in the transition of care from the hospital to the home and decreased 30-day readmissions of high-risk patients. Since the program was introduced, it has been deemed the most successful project to reduce 30-day hospital readmissions in the Franciscan system and has provided more than $5 million in medication assistance to patients.
THE PATIENT SAFETY SERVICE & LEADERSHIP AWARD
Dr. Tim Hobbs, senior advisor to clinical transformation at Community Health Network, received the 2018 Patient Safety Service & Leadership Award for his exemplary dedication to quality and patient safety. Hobbs was honored for hardwiring processes that have engaged caregivers in developing system-based improvements to advance patient care.
THE PATIENT SAFETY PARTNER AWARD
The Indiana State Department of Health (ISDH) received the 2018 Patient Safety Partner Award. ISDH’s vision is to promote and provide essential public health services for a healthier and safer Indiana, as well as promote healthy personal choices with a focus on disease prevention in Hoosier hospitals. IHA has partnered with ISDH on a number of successful initiatives, including the creation of 10 public health preparedness districts across the state, the establishment of the Indiana State Trauma Committee, and the work of the Indiana Perinatal Quality Improvement Collaborative.
